Team Leader - Aged Care & Community Services

An exciting opportunity exists for an experienced Physiotherapist to join our team as Team Leader, working in a hybrid role across both Aged Care and Community Services primarily based in Traralgon but also servicing surrounding communities. This opportunity is well suited for those keen to work across both clinical settings in a leadership role.
We are seeking an experienced Physiotherapist, ideally with experience in either Community Services and / or Aged Care. Leadership experience is preferred, but we happy to provide training and support those who may be taking first step into a leadership position. The right attitude and willingness to learn is a must!

Here at UPG, you will find a group of physiotherapists who are passionate about delivering exceptional care in across multiple health care settings. Broadly, our group works in aged care facilities and in the community across VIC, NSW, QLD, TAS and WA improving the quality of life for thousands of elderly clients. In addition we have a Flagship Studio where we provide Pilates, Yoga and Private Physiotherapy service along with sports physiotherapy services to local clubs based in Melbourne. As a member of the UPG team, you will have an incredible opportunity to apply youir skills and knowledge across a variety of settings.
Responsibilities and Duties:
Leading your team by example, setting high standards of communication with clients
Coaching and Mentoring your team to support their learning and development
Managing your daily schedule effectively including clinical and non-clinical tasks
Participating in ongoing Professional Development and developing skills through individual mentoring and team learning events
Aged Care
Completing comprehensive assessments, formulating plans and delivering treatment to clients
Correspondence with residents, families, nursing, medical practitioners, allied health practitioners case managers and equipment suppliers
Exercise prescription and review, including leading of exercise groups
Liaising with the broader clinical team and facility management to deliver tailored care plans and assist clients to achieve their goals
Delivering Falls prevention workshops, Manual Handling and other education to staff
Community Services
Engaging with clients with a variety of clinical presentations across a range of community environments from clients home, local pools and retirement living villages.
Seeing clients across a variety of funding streams including NDIS, Home Care Packages, DVA, STRC, Primary Care and CHSP.
Liaising with Case Manager/Support Coordinators and the larger team to enable clients to achieve their goals.
Managing your schedule effectively including clinical and non-clinical tasks
Qualifications and Skills
A Bachelor of Physiotherapy OR an equivalent internationally (and APA) recognised qualification
AHPRA Registration (General Registration)
Clinical skills to perform comprehensive Assessment, Treatment, Risk Assessments
Appropriate working visas - no visa restrictions
Valid Driver’s License and Access to own car
A National Police Certificate
NDIS Check/Working with Children Check
Strong interpersonal and communication skills
Ability to work in a team
